TV Yellow truss rod cover

I recently saw a pic of a Double Cut TV Yellow Special and the truss rod cover had Les Paul in script lettering and TV SPECIAL or TV MODEL below it.
I'm trying to find another pic of it, and ultimately find a place to have one reproduced, or try to find an original (which I know will be difficult).
